Campbell Ave, Suite J 70, Campbell, CA 95008. The above address is a link to a google map to the clinic. You can also get driving directions from google. The clinic is in building J, suite 70. You can drive into the Campbell Community Center from either Winchester Ave or Campbell Ave. To find building J, drive towards the tennis courts at the back of the community center, building J is the last building before the tennis courts. There is plenty of parking available. Hours:   My current hours are:      Monday from 1:30 PM to 9 PM                                                           Thursday from 1PM to 9 PM                                                            Friday from 10:30 AM - 4 PM To schedule an appointment to see me in the Campbell please call my receptionists on my toll free number: 1 866 863 5560 My receptionists are available 24 hours a day and seven days a week to schedule an appointment or change a scheduled appointment time. If you are a new patient, tell the receptionist so that they schedule you for the appropriate amount of time.
